Words cannot capture how we feel at this moment, with our healthy, happy four day old son in our arms. I have dreamed about this moment for so so long. And for much of that time, this dream did not seem achievable. We struggled to have a baby for several years, and every treatment attempt led every doctor to give us a discouraging outlook. Then we found Dr. Shivani and her clinic, it was the first time we were truly greeted with understanding for what we had been through, with true compassion for what we had lost, and with positivity around the fulfillment of our most important dream.

Many asked us why India, why this clinic and I have always said the same thing: India has a process and this was the KINDEST clinic we could find. Dr. Shivani, you helped us through some difficult times, and you have helped to TRULY heal me. The birth of our son has made me the HAPPIEST and MOST PEACEFUL I have ever been. We are elated, but after years and years of anxiety and stress and sadness over my unfulfilled desire to have a baby, it is the peace and calm this baby brings to me that I cannot believe. My baby has healed my broken heart. And, he would not be here without all the love, care and attention that Dr. Shivani and her staff have given to us, to our generous surrogate, and our fantastic egg donor. Your international coordinators, Meg and Margarida, were a lifeline during the throes of "surrogacy pains", which, although not as painful as "infertility pains", were quite significant. Also, your medical, administrative, concierge and financial staff were all so professional, kind, respectful and available. For someone who is quite private and did not know anyone who had travelled a similar path, it was sooooooooooo nice and essential to have this team. Each and every member of the team responded to my emails with such reassurance and promptness. It made having a baby on the other side of the world the most normal, inclusive and supportive process ever.

Everything I have written does not truly capture the essence of how I feel. Just know that I am forever grateful to our surrogate, our donor, Dr. Shivani and the team at SCI. The only way I know I can repay you and God for finally giving me the baby I have wanted with every fibre of my being is to love and care for my son with every inch of my heart and soul. Since, he is now my heart and soul I know that this will be easy.

We appreciate the challenges and anxiety that goes with having a pregnancy half a world way; and as we like to remind those who are just starting their journey into surrogacy, this relationship with SCI ( or any clinic) is one that will last 12 months or longer for those considering sibling projects.

A year has passed since we last met and our thoughts are often on the two months we spent in Delhi. At the time we felt scared, confused and unbelievably happy. In retrospective we see that you and your staff helped us through the whole ordeal in a warm and caring manner that made us feel safe and well looked after. Thank you so much!

Little Karlise has grown into a beautiful toddler. She is a humerous girl with an adventurous personality. Everything can be climed, eaten or explored and she greets the world with smiles and laughter. In August she started kindergarden and it was a success from day one. Karlise loves to play with other children and she has become very popular among the staff. A true heartbreaker:-)

We have enclosed a few photoes so you can see for yourself.

Twins - what would you tell prospective parents?

Given the recent flurry of twins and triplets at SCI, we thought it would be wise to share some of the wisdom from those who have brought their babies home.

We thank Nan from Australia who was very kind in sharing her thoughts along with these beautiful photos of her babies

"My goodness where do you start to talk about such a wonderful rollercoaster journey. As the IP of twins, life is about to become hectic I thought that I would keep this simple with a few thoughts after four months. I hope it helps.
·         If your twins are born early and are admitted to Delhi New Born; it’s a long way away. Drive there with Rahul or one of his drivers
·         Dr Gupta ( Delhi New Born) is a wonderful clinician and all babies admitted to his care are very fortunate.
·         Once discharged and you are still in Delhi hire a nurse around the clock. It will cost about 1000 INR for a 12 hour shift ( roughly $20). You can sleep and take advantage of the nurses calm experience. Ask them to teach you how to swaddle.
·         Stay at an apartment. You will be able to boil all bottles and have room to make a home for your family.
·         Tulip apartment in GK 1 was fantastic, comfortable roomy and close to where you need to be.
·         Get everything delivered; delivery is very cheap and makes like a lot easier.
·         Enjoy being in Delhi; when you get home you will weirdly miss it.
·         Buy some Indian colic medicine for babies its fantastic.
·         Buy some Indian panadol it is cheap and effective.
·         Buy the formula in bulk.
·         Take two phil and ted cocoons with you; they have been fantastic. My twins are still sleeping in them.

·         You can’t have too many bottles, bibs, and singlets, light muslin wraps for swaddling.
·         When you come home take all offers of help.
·         Sometimes one twin may have to cry for a moment when you are on your own and its twin is crying harder.
·          When you come home seriously consider getting a night nurse to help at least one or two nights a week. No sleep is a form of torture. In Sydney I used Dial an angel and they were fantastic.
·         Love your babies
·         Think about having some more "

Clients come to us at SCI with many stories of infertility and heartbreak. One of the most common stories for heterosexual couples is a history of failed IVF treatments.

It gives us great delight when, after such a history, a couple becomes pregnant with their first attempt and 9 months later welcome the birth of a much wanted child. Below is one such family, we thank them for sharing their story

Dear Dr Shivani and all the SCI Team,

Thanks to your professional and highly knowledgeable staff for helping us have our first child. Our son is very healthy and is doing well. We wanted you to know what a wonderful experience we had with SCI. After years of unsuccessful IVF in Australia; this was the first attempt with your clinic and now- nine months later we are welcoming our child into the world.
Throughout the entire process (IVF to birth) we always felt confident that we were receiving the best service possible. Your staff are highly professional and skilled at what they do. They also have excellent interpersonal skills and were always very quick to respond to any emails and provide regular updates in regards to our surrogates pregnancy. We were reassured to know how well our surrogate and her family were looked after and supported throughout the journey.
Despite being so far away – we felt confident that any questions would be answered quickly-day or night. Upon return to India to greet our son into the world- we have been amazed at how much support has been available (night nurse, driver, accommodation assistance, and administration paperwork). We were also impressed with the nursing staff at the hospital who ensured we were confident with all baby care tasks before our son was discharged.

This has been a fantastic experience for us- and words are simply not enough to thank you for our new born son. We hope to return in the future and try for another child to compliment our new family.

The words "Thank You" don't quite seem enough for what you've given our new family. In fact, you've given us a family, and there really aren't enough words to describe the gratitude we feel toward you and your team, and the sheer joy you've brought into our lives. Everyday since their birth on Monday, we wake up, after very little sleep, feeling like we just hit the jackpot, especially after getting twins on the first try.

When we first started looking into the specifics of surrogacy, we were hesitant, to say the least. After meeting with some local clinics in the United States, we became a bit discouraged. And more so after looking into international options...until we came across countless wonderful comments about SCI on so many surrogacy blogs. But even though we were confident in your skills to help us start our family, we understood the odds of success.

Well, over these last 37+ weeks, you and your staff have lived up to all of our hopes, and each of the comments that lead us to SCI in the first place. You've all done a fantastic job - from your clinical efforts, timely updates, finding our great surrogate (we're still amazed that such a small woman was able to carry twins for 37 weeks), to the team that helped us coordinate all the legal and necessary procedures to get our babies' passports and exit visas, and even the car service you work with (we just love Rahul and his team).

Diwali* or Deepawali is one of the most popular festivals of India and of the Hindus. It is believed that the festival of diwali commemorates the return of Lord Rama to Ayodhya , after 14 years when Rama return to Ayodhya, his capital city, people were over joyed by the news of his arrival and started the tradition of Diwali by celebrating the day with lights and lamps.

Many Hindu consider Diwali as the beginning of new year, in the traditional Hindu Business world, it marks the beginning of a new accounting year.

Diwali is such a wonderful festival, a time of giving and sharing, a time to catch up with people, in other words its time to catch up with the little joys that we keep overlooking for the remaining part of the year. Its time to visit friends and relatives, so like Christmas, people take leave, armed with sweets and savouries meet and spend time with those closest to them with houses brightly decorated with Divali lamps, candles and hangings.

What happens after egg collection or what you need to know about embryos?

Once you have egg collection we, at SCI, do our best to make wonderful healthy robust embryos. So, as part of what we offer at SCI includes ICSI.

Intracytoplasmic sperm injection (ICSI) is a procedure in which a single (best looking sperm) is injected directly into an egg. The skill of the embryologist is paramount here and we are fortunate to have one of India's leading embryologist to do this.

What happens next : Embryo Culture

SCI follows the Hammersmith protocols in which embryos are cultured until having reached between an average of 2–8 cell stage between day two and three days after retrieval. Our embryologist will then grade the embryos (based on the number of cells, evenness of growth and degree of fragmentation), that will assist Dr. Shivani, in deciding which embryos to transfer and how many to freeze. Dr. Shivani, as your treating doctor will undertake the transfer to either you or your surrogate.

With SCI, we believe that IP's should have as much information about the process as we are able to provideso that IPs can :

1. Be informed
2. Have all the information available so that they can make informed, evidenced based decisions in moving forward.

Please see a sample embryology report given to all clients at SCI, (with client name removed). From this report you can see the number of egg collected, and the maturity (in this case 18 eggs, 17 mature - MII and one GV - germinal vesicle, not mature. It is important to know not just how many eggs were collected, but how many were viable.

From this report you can see the number of viable eggs that had ISCI and were fertilised ( again not all eggs fertilise) - in this case 17 - 2PN is 2 pro-nuceli - the DNA from dad and DNA from the donor.

On the left of the form, the four best embryos - all 8 cell of grade 1-1.5 were transferred by Dr. Shivani to the surrogate. If you would like to see Dr. Shivani undertaking an embryo transfer, please click here : http://www.youtube.com/watch?feature=player_embedded&v=hh2AugTW1Eo

On the right side of the form, you will see the number of embryo frozen and the grade (7 x grade 1 of 6-8 cells, and 3 x grade 2-3 of 6-8 cells.

Our embryologist doesn't freeze anything lower than a grade 3, as there is so little chance of the FET leading to a sucessful pregnancy and live birth. This is important information to have, especially if you are proceeding with FET in the future. Some less reputable agencies will often transfer lesser quality embryos with very little chance of success.

This was the first try for these clients, with wonderful results (and a twin pregnancy) , they are in the lucky 70% of clients with donor and surrogate who become pregnant on first try. Of course, there is always the unlucky 30%, but most SCI clients are pregnant on a subsequent transfer with 90% of clients becoming pregnant within two attempts.

Egg Collection

We have many women who choose to self-cycle with SCI, some of whom very generously share their stories via their blogs, but I thought it would be helpful to talk about the process. With SCI, it is important for you to note that it is Dr. Shivani who has trained and has extensive experience in all forms of egg collection will be the doctor who monitors you cycle and undertaking your egg collection.

Before Egg Collection

Prior to any egg collection it is important to have a detailed assessment of a woman’s fertility hormone levels and that we track your cycle to ascertain when you are ovulating. Establishing your AMH & FSH levels will give us some evidence based criteria to which we can assess your chances of success.

IVF egg collection

During a typical menstrual cycle, one egg matures and is released during ovulation. In order to increase the chances of creating a viable embryos and a successful pregnancy, we undertake a controlled hyper stimulation in order to stimulate resting/ dormant follicles. Your treatment is based on the information provided prior to egg collection, based on your hormone levels and ovulation cycle, so no one person’s cycle is the same as another.

Egg retrieval

Eggs develop in fluid filled structures in the ovaries called follicles. You cannot increase the number of your follicles that you have, but you can ensure that you make the most of what is available by carefully tracking your cycle and ensuring that medication is given at the right time. Each follicle can be seen and measured by ultrasound and contains one microscopic egg. The egg is loosely attached to the follicle wall. When the follicles are mature (determined by ultrasound measurements of follicle size and hormone measurements), the egg aspiration procedure is done to take the eggs out.

Anaesthesia for egg retrieval

Anaesthesia medications are given through an IV so that the woman is "out" during the egg retrieval procedure.

A needle is passed through the top of the vagina under ultrasound guidance to get to the ovary and follicles. The fluid in the follicles is aspirated through the needle and the eggs detach from the follicle wall and are sucked out of the ovary ( please watch this video to see Dr. Shivani undertaking egg collection: http://www.youtube.com/watch?v=q0oG-Gd7tiE&feature=player_embedded#!.

Laparoscopic egg collection

Is when egg collection occurs via you abdomen. This is an option for women who have had hysterectomies or other health conditions in which it makes it difficult to access your ovaries’. It is important to note that for women who have had hysterectomies, ovaries do move about and it is important to have an experienced doctor such as Dr. Shivani to undertake such procedures.

Terry Clayton and Steve Oxley had to travel halfway around the world to get their sons, but the journey was worth the effort

Terry Clayton and Steve Oxley knew early on in their relationship that they wanted to be parents. And after a little research, they decided that commercial surrogacy was the best way to go.

Still, as Clayton notes, surrogacy wasn’t without its potential pitfalls.

“We were certainly concerned about the perils of commercial surrogacy. You hear these stories about the mother changing her mind at the last minute. And commercial surrogacy isn’t legal everywhere. It is legal in some states, but not others,” he said.

The couple, who lived in California at the time, also quickly found another drawback: cost. Although commercial surrogacy is legal in California, Clayton says, they discovered that going through a reputable agency would cost them about $100,000 to $125,000 for the full package of services.

“That just wasn’t affordable for us,” Clayton says.

But he and Oxley weren’t ready to give up on their dream. So they started researching the issue and quickly found online a large community of people who either already had children through commercial surrogacy or were in the process of having children that way.

Clayton travels frequently in his job as senior vice president of sales with a California-based furniture company. He was on a business trip early in 2009 when he found out, through that online community, that commercial surrogacy is a thriving business in India, where the service is available for about a quarter of what it would cost in California.

“I called Steve and said, ‘What do you think?’ And he was all for it,” Clayton recalls. “So I emailed a clinic I found in Mumbai. I chose them because of the price — about $26,000 not including the travel to India — and because they had a really nice website and they specifically marketed themselves to the LGBT community.”

And so the process began. Clayton and Oxley started by choosing an egg donor after studying profiles on the available donors, including their height, weight, medical history and religion. The profile on each included a “tiny head shot,” but no names. Donors, Clayton says, remain “completely anonymous.”

Clayton adds that in looking through the profiles, he and Oxley noticed that many of the women “looked unhappy.” So they decided to find one who “looked pleasant, happy” and who was relatively tall.

Clayton explains that while they chose the egg donor, the biological mother of their children, the clinic itself chose the surrogate, the woman who would carry the children. Having separate women as the egg donor and the surrogate, Clayton says, helps cut down the possibility of an emotional attachment between the women and the child.

The couple also had to decide which of them would be the biological father of their future child. For Oxley, that decision was an easy one.

“I have [siblings] who have children, so my parents already have grandchildren that they have biological ties to,” Oxley says. “But Terry is an only child. I thought it would be nice his parents to have grandchildren that they were connected to biologically.”

With that settled, Clayton began a series of tests that included “a lot of bloodwork,” an HIV test and an analysis of his sperm to ensure he was a good candidate for the procedures. Then about a month later, after paying the full fee up front, Clayton boarded a plane to India.

Oxley stayed home on that first trip to help contain costs, the men explain.

Physicians at the Mumbai clinic retrieved the eggs from the donor mother and the sperm from Clayton, and fertilized the eggs in vitro. The fertilized eggs were then implanted in the surrogate.

The surrogate, Clayton says, got pregnant on the first try, and he came home to California to wait. And then, one day shy of 12 weeks later, they got the bad news. The surrogate had miscarried.

Clayton wrote about the experience on the blog he had been maintaining about their efforts, and soon found out that other couples who had used the Mumbai clinic had been equally disappointed.

“I found two other couples online who were commiserating about that clinic, about how poor their communications were,” he says.

Oxley adds, “We got an email saying she [the surrogate] had miscarried. An email!”

And although the contract said they would receive a refund if the surrogate miscarried within the first 12 weeks, they ended up getting only about a third of their money back.

The men knew they still wanted to have a child, but the sadness of losing the first child made them delay. Eventually though, through an online contact in Australia, they heard about Dr. Shivani Sachdev Gour, a London-trained physician who had just recently opened Surrogacy Centre India in New Delhi.

“She put together a great package for us,” Clayton says, noting that the price this time was lower — only $23,000 — and that instead of having to pay the full price up front, they made payments as the process went along.

So again the couple went through the process of choosing a donor and again Clayton traveled to India. This time, Dr. Shivani took two sperm samples, separating the sperm into four vials and freezing three of them so Clayton wouldn’t have to make another trip if the first implantation effort failed.

This time the surrogate again got pregnant on the first try, but she miscarried at seven weeks. At this point, Clayton says, “We decided we needed to take a breather. We were racking up quite a bill, with all the travel and everything. And we decided to wait a bit before we tried again. We wanted to make sure we were ready, emotionally and financially.”

In the meantime, the couple had decided to move to Texas, where they would be closer to Clayton’s parents and where they could more easily afford a larger home for their soon-to-be-growing family.

When they decided to try again, they chose a third egg donor. Even though the woman’s profile information had been inadvertently left blank except for her photo, Clayton and Oxley knew she was the one they wanted.

“The second we saw her, we knew she was the one. She was 5-foot-5, the tallest of any we had seen, and she looked like an Indian version of my mother,” Clayton says.

The new procedure started out well. The donor produced 32 eggs and almost all fertilized. Four of the fertilized eggs were implanted, while the rest were frozen for possible future use.
Of the four implanted eggs, three took, and the surrogate was pregnant with triplets.

In the U.S., Clayton explains, surrogacy clinics are only allowed to implant two fertilized eggs at a time. In India, doctors can implant four to increase the chances of a successful pregnancy.

However, if more than two of the embryos are still viable at 11 weeks, doctors have to perform what is called a “selective reduction,” since surrogates are not supposed to carry more than two babies to term. That’s what happened in Clayton and Oxley’s case.

“It was awful, just awful,” Clayton says with tears in his eyes. “I can’t stand to think about it even now.”

From that point on, though, the surrogate had “basically the perfect pregnancy,” Clayton says. “If she complained that her toe hurt, they took her to the hospital. They took very, very good care of her.”

At the 36-week mark, Clayton and Oxley packed up and headed together to India. They were able to go with the surrogate for her last ultrasound appointment, and a c-section was scheduled for the next week.

“We had rented an apartment, but when they told us it would be the next week before the babies were born, we decided to go away for the weekend,” Clayton recalls. “It had been so stressful, and we just wanted to relax those last few days.”

But the babies had other plans. The daddies-to-be packed for their weekend trip then went to bed about 3 a.m. At about 6 a.m., Dr. Shivani sent a text to let them know the surrogate was in labor and about to deliver.

By the time they woke up, saw the text and made it to the hospital, their twin sons — Ajay and Jag — were about two hours old.

“Terry held Ajay first and I held Jag first,” Oxley says. “It’s interesting, to this day it seems each of us still has a special bond with the one we held first. We both love them equally, but there’s a bond there with the one we held first.”

The boys’ names, Clayton explains, are Hindu. Ajay means “invincible,” and Jag means “the universe.”

“We wanted them to have a connection to their mother’s heritage, even though we don’t even know her name. We are both Catholic, but we want them to be exposed to a variety of cultures. We want them to connect with their Hindu heritage, too.”

So they had their sons, at long, long last. But Clayton and Oxley still had more hoops to jump through to get their twins home. They spent a total of five weeks in India, paying “expediting fees” to get the babies’ birth certificates and the exit visas necessary to take the children out of India. The government also sent an investigator to interview the couple and the surrogate who delivered the twins before Clayton and Oxley could bring their sons home.

Finally, though, they made it, returning to their new North Dallas home in the first week of October.

Today, Jag and Ajay are just shy of their first birthday, and Oxley’s second-parent adoption of the twins was finalized at the first of June. Oxley, who worked in property management in California, stays home these days to be a full-time dad, while Clayton still has to travel often for work.

It’s hard to be away from his family so much, Clayton says. But he uses the phone and the Internet to be close to them, even when he isn’t physically present. And the grandparents on both sides of the family are thrilled with their new grandchildren as well.

On a recent Friday afternoon, Oxley’s mother was already at their home, and Clayton’s parents were on their way back, for the second weekend in a row.

But the change for Clayton and Oxley is profound.

“We have a family now. As the years go by, we won’t be just two old men growing older, we’ll have a family. We’ll have our children and our grandchildren,” Oxley says.
Clayton adds, “It took a while. We had to go through a lot more than straight couples. But we wanted children and now we have two beautiful sons.

“Everyone deserves to have a family,” he says with a proud smile, “even if it looks a little different.”

Working within the UK requirements for Intended parents

Having completed a core component of my training in IVF in the United Kingdom, not only am i privileged in having the experience of having working with colleagues’ and clients in the UK, but these colleagues were one of my first referral points for International clients undertaking surrogacy in Delhi.

As such, SCI Healthcare has gained considerable experience over the last 2 years with working within the requirements as designated by the British Government.

We have a surplus of “single” surrogates (i.e. widowed or divorced women who have had trouble free pregnancies). We have no need to recruit or advertise, so there is no wait time for surrogates.

We are knowledgeable on all the documentation required by the British High Commission in Delhi and are keeping up to date with new requirements as they happen, such as Birth Certificate for the surrogate's children etc. and are able to provide these necessary documents at the appropriate time

Our most recent success has been meeting the exacting requirements of the EU to transfer human tissue samples out of the UK to Delhi.

Our Client Relationship Manager and Surrogate Co-ordinator are experienced in supporting our surrogates through the interview process required by the British Government. The process of going home with your baby is bureaucratic and can take up to 12 weeks, but IP’s are not left alone.

It is with great delight that we announce that SCI Healthcare is expanding. Those of you about to go to Delhi would be familiar with these new names, but let me share them with the rest of our community.

ISIS Hospital is our brand new ART (Assisted Reproductive Technology) Hospital that belongs to, and is a crucial part of SCI.I am over the moon to able to have my own hospital, it is my pride and joy I have already had the pleasure of undertaking over 60 procedures at the new site with a number of positive pregnancy results. This is a new state of the art facility which meets the highest medical standards and is aesthetically beautiful, comfortable, secure and caring enviroment for our IP's & surrogates. Along with the new SCI offices ( 5 minutes away by walk from ISIS in Kailash Colony) this will provide the cornerstone of SCI Healthcare. We are also taking the opportunity to move the site of our baby births to Paras Spring Meadows Hospital. This facility part of a chain of hospitals in Delhi NCR which is closer to our new office and across the road from ISIS. IP's will benefit from more space for parents, an excellent NICU and LDR (labour delivery recovery rooms) and a new born educator that teaches Kangaroo Mother care and other parenting techniques.

A blessing of multiples with SCI

It is with great joy that i write that we are very busy with babies at the moment. The Nursery is overflowing with much joy, laughter and a touch of anxiety as parents hold and feed their babies for that first time.

In the last fortnight we have had one set of triplets and four sets of twins born to parents from the U.S and Australia, with two more sets to be born any day to parents from the U.K and Australia.

SCI service to these families includes our concierge who assists with accommodation and transport, access to day and night nurses from our experienced pediatric team and access to baby equipment (when available)such as baths and sterilizers.

These babies are going to families from across the world, rich in diversity, we would encourage you to read the blogs of those families courageous enough to share their stories.

Sending Embryos & Sperm to Delhi - no problem, no issues, no country wide ban!

I have received a number of emails asking for clarification on the issue of a supposed ban on transferring embryos & sperm to India.

I can categorically state that there is no ban across India. We at SCI have and continue to receive both samples of sperm and embryos. In the last few weeks we have received both embryos and sperm couriered with the assistance of two companies from the U.S.A, Canada and Australia.

Again there is no country wide ban.

It is our understanding that there have been some issues with entry into Mumbai and a 'entry tax'. From the courier companies that we work with,they have stated that this has caused a number of issues for both IP's and the courier companies. Until this issue is resolved most courier companies are choosing not to go to Mumbai.

In the last three months we have been receiving almost weekly shipments from around the world,with many couples now sucessfully pregnant. We are planning to receive many more cyro shipping containers in the coming months.

We have been very busy and blessed in the end of November. Seven babies born in Seven days to parents from Australia and the United States of America.

In this picture you will see baby Ruby( mentioned in the last Blog post) being held by the very popular and able Shilpi, and a beautiful pigeon pair of twins (being held by one of our nurses), born to Australian parents.

One of the blessings of my position is seeing the joy ( and anxiety) of new parents when they hold and feed their baby during those first few days in hospital. It is a blessing that has been abundant over the last number of weeks.It is something that no-one at SCI will ever get tired of witnessing, nor take for granted.

"My baby Ruby was born on November 17, 2010, happy and healthy, thanks to the loving care that I received from Dr. Shivani and her staff.

Every step of the way was warm and reassuring. From my initial contact to the day I took Ruby home, everything was perfect. I had worked with another clinic in the past and unfortunately miscarried at 4 months. Dr. Shivani was recommended by friends whom I met at the other clinic (who were also unsuccessful, and are now very close!); they said they had had a great experience up to that point. So after doing some research and some hand holding from Megan, I flew to India last March.

My entire experience then was fantastic. Shilpi called me daily to make sure I was fine. Dr. Shivani was great; very, very warm and informative. I left India feeling great about the entire process. Two weeks later, I received an e-mail from Dr. Shivani that the surrogate was pregnant (I used an Indian donor and surrogate). Each month, I received ultrasound photos and reports and reassurances that everything looked fine.

Ruby was born a month early which was a bit of a surprise, but seeing and holding her at Phoenix Hospital was the best moment of my life. I took her "home" three days later and everything is wonderful. She's simply beautiful.

I cannot say enough good things about Dr. Shivani and her staff. The nurses at the hospital were wonderful. Megan and Shilpi are fantastic. And Dr. Shivani was a caring and loving presence through the entire pregnancy and beyond.

I should add that every step of my initial agreement was honored -- there were no financial surprises, unlike what I have heard about other clinics. SCI Healthcare, in my experience, was the epitome of integrity. I was always made to feel like Ruby (formerly known as "the baby") and I were the two most important people in the world.

NEWSFLASH- Surrogacy insurance with a certificate of currency

SCI have spend the last number of months negotiating with the premiere insurance companies in India to provide life insurance for our surrogates (LIC etc).

Insurance premium will depend on age of the Surrogate mother and other relevant life factors and will be between Rs 5000 - Rs 9000 for a year with the payout being Rs 1,000,000

Our reputation and huge numbers of pregnant surrogates (touchwood) have helped with negotations.

We understand that Intended parents want what is best for their surrogates and their families. Now, IP's have the additional option of insurance which can be purchased when heart beat of your baby is confirmed.

Copies of certificate of currency will be provided to IP's.

SCI – How we maximise every opportunity for you to have a baby - part two

We are very aware of the financial considerations that impact on families undertaking surrogacy and are aware that families have made considerable sacrifice to undertake this journey with us at SCI.

SCI would wish that everyone becomes pregnant during their first attempt, but we know that this is not always possible. We are very sensitive to the difference between becoming pregnant and having a live birth. HCG levels (positive pregnancy test), alone are not a good predictor of you having a baby in your arms nine months after these results- but it is a wonderful first step.

We will not suggest that you are pregnant unless you have a solid HCG level (ideally over 50). We do not confuse chemical pregnancies with viable pregnancies. We look for evidence that your pregnancy has every chance of resulting in a live birth. What the evidence and research tells us, is that the first significant milestone towards this goal is seeing your baby’s heartbeat on an ultrasound. Clinically this puts a pregnancy into a lower risk category for miscarriage. For this reason, our second stage payments begin, not with a positive pregnancy result, but with a viable heartbeat.

We don’t believe that IP’s should lose a significant proportion of their budget with non-refundable payments if a miscarriage occurs before heartbeat with evidence stating that the highest risk of miscarriage occurs between receiving a positive pregnancy test and having an ultrasound that confirms heartbeat.

At SCI, payments are made bi-monthly for the duration of the ongoing pregnancy, rather than a trimester basis. If you are unfortunate enough to experience a miscarriage in the second trimester, you are not required to pay the balance of your surrogacy fees for the second and third trimester, rather you are asked to pay the fees for the month, with an additional compensation which is given to the surrogate for following month.

We have a separate financial contract included as an attachment to the main contract in an attempt to be as transparent as we are able to be, stating what is and is not included. We acknowledge that this can be challenging, as no one can predict how any one pregnancy will progress. For example: We do not charge ‘extra’ for twins, but acknowledge that twin pregnancy are higher risk and may require more medical care, this is billed to the IP’s as it occurs and is discussed with the IP at the time it occurs.

Another example would be a Caesarean birth. If your surrogate requires a Caesarean birth, the amount paid covers not only the medical procedure and hospital stay while at the hospital, but also needs to include:

follow up medical care for the 12 weeks ( of more if required),

additional medication,

additional transport,

additional stay in the studio apartment for a further 2 -3 months,

additional home help and assistance as the surrogate is not able to carry or lift any weight, additional food & supplements,

additional visits by family members, nurses, social workers, community workers, cleaners and cooks,

and follow up visits by the nurse/social worker when they return to their home.

It is not a simply a medical procedure at a hospital, nor is the medical procedure all that needs to occur to ensure the health and well being of your surrogate.

The aim for SCI always remains for IP's to take a baby home with them. We are aware of the financial considerations that face all families and ensure at all times that we focus on not just getting pregnant, but having a viable pregnancy and live birth.

SCI – How we maximise every opportunity for you to have a baby - part one (updated)

We, at SCI understand both the emotional, physical and monetary costs of all our families undertaking this journey. Our research tells us that we are in the middle of the road when it comes to our costs and fees, so we thought we would take the opportunity to share with you what we do to maximise every chance of you having a baby.

We start at the beginning; we have many couples who choose to self cycle, we are aware of the evidence through hormone levels (especially AMH* levels) that are predictors of the likely hood of a viable pregnancy and live birth. The heart breaking facts of diminished Ovarian Reserve are that a women can still produce eggs, they can still develop into wonderful looking embryos ( but not always), but the chances of getting pregnant are greatly reduced and if you do get pregnant the chances of miscarriage are very high. This is why we insist on all clients who are looking at self cycling and egg donors undertake this test. We are aware that some couple choose to proceed with this information, but we believe that clients should make informed decisions. Sometimes we are in the unfortunate position to being the first to tell unsuccessful egg donors ( Indian and Non- Indian) of their poor fertility outcomes.

Sperm quality is something that is not often talked about and presumed to be “OK”. With a sperm culture and analysis, this assumption can be tested. Poor sperm quality can have as significant impact on becoming and staying pregnant as egg quality.We want all couples to be aware of this. Excellent embryo grading is what we all want, with men we hope for average or better sperm quality, but one of the most common age related factors is poor motility. One of the many ways we assist with this is making ICSI** a part of our package, so all clients benefit.

All these discussion are difficult to have with clients, but we do not want to give anyone false hope by undertaking treatment ( or multiple attempts at treatment)with a likelihood of a poor outcome.

We are in the fortunate position (through good practice and management) of being one of the few agencies that does not need to advertise for surrogates and having a surplus of screened surrogates at any one time. Through word of mouth from past surrogates and their families, we have women and their families approaching us daily for the opportunity to become a surrogate. Potential surrogates often come with their own mother, husband and extended family to enquire about this opportunity. All surrogates (and their husbands) are screened by our medical team and our psychologist.

We look to the evidence which guides us in best practice and therefore best outcomes for these women and their families and as a consequence, our IP's. As previously described:

“SCI, we do not believe that surrogates should be separated from their
immediate families, nor do we believe in dormitories for accommodation. This
is backed by research which tells us that surrogates have poorer health outcomes,
increased anxiety and greater recovery times if separated from their immediate
family. Mr. Singh provides us with spacious Studio Apartments that are fully
furnished; have cooling to cope with the Delhi heat as well as fridges and cable
televisions in each apartment, the cost of which is met by IP’s.
..The teams (of Social workers/supportworkers/cleaners) meet with surrogates and their families on a daily basis at the surrogate housing. They look after the daily
up-keep of the studio apartments and ensure that the surrogate and their
families have all their needs met. Fresh Food, drinkables and supplements are
provided every second day for all in the apartment. If a surrogate chooses to
have her children with her (often the case for younger children) then a second
family member must be available to provide care..”

We believe because we treat the surrogate as part of her family and her community, not as an individual or a body part, extending this care to the surrogate’s children and other family members, is one of the many reasons for the successful word of mouth campaign that has allowed us to be in the privileged position of having more surrogates than we are able to use. A proportion of the fees that you pay cover these additional items that ensure a higher quality of care for the surrogate and her family.

This includes, but is not limited to individual air conditioned studio apartments rather than dormitories, individual cable TV’s so surrogates or their children can watch what they want, fridges for consumables, school fees, with uniforms, textbooks and school supplies, food & supplements for extended family members, recreational activities for all, additional child care, medical care for the surrogates children, children’s carer and extended family, transport for extended families to visit the surrogate etc.

This makes up our costs, but is of course separate to what the surrogate actually receives in compensation which is paid throughout the surrogacy process . We are very aware that this word of mouth holds us to account, if we fail to provide the quality of care, opportunities or outcomes as promised or described, this surplus of potential surrogates would cease to exist.

For IP's, this means that we have healthy emotionally robust surrogates, with supportive families who want to become pregnant. As with best practice in relation to surrogacy, our choice of surrogate occurs three to four weeks prior to egg collection. We aim to match as closely as we can, the cycle of our self cycler/egg donor with our surrogate.

This usually means that we are monitoring the cycle of a number of potential surrogates to find the best match possible. A proportion of your fees go towards compensating the potential surrogates who do not successfully match.

But even with this care, there are times when the surrogate cycle is not optimum, the uterine lining may not be thick enough or too thick, the hormone levels may not be quite right or there may be some other personal factor that impacts on the likelihood of a successful outcome.

If this occurs, we simply do not proceed. Unless planned, we do not offer a back up surrogate because we know the importance of matching cycles. At times, this may mean that your egg collection is delayed, in rare cases this may mean that your embryos will be frozen, but we will not transfer to a surrogate unless we believe that there is every chance of the surrogate becoming pregnant and having a live birth.

At SCI, we will do everything to maximise all opportunities for you to have a sucessful outcome,what we all want, above all else is for you to go home with your baby in your arms.

*AMH - Anti-Mullerian hormone is a hormone produced by the granulosa cells of the early developing antral follicles. These are the immature eggs that wake up from their dormant state and develop into mature eggs. As a woman runs out of eggs, the number of these small antral follicles decline in number and as a result the serum Anti-Mullerian hormone falls. This is why serum Anti-Mullerian hormone testing is a good estimate of residual egg number. Women with diminished ovarian reserve have diminished fertility and an increased risk of miscarriage.

**Intracytoplasmic sperm injection (ICSI) is an IVF procedure in which a single sperm is injected directly into an egg.

I would like to take the opportunity to introduce you to some of the hidden members of the SCI team. Clients who come to Delhi know me, Dr. Shivani, as well as much loved Client Relationship Manager, (Travel &Tourism) Shilpi Sharma. Some clients may meet part of our Administrative team ( including our International Client Managers), but we are much bigger that this small handful of people.

We, the SCI Team are made up of over 45 members of staff working to provide a wrap around surrogacy service, as a single team to fulfill the dreams of our clients to become parents. The majority of our team are based in Delhi, although we have International Client Managers based in Australia and America. We are dedicated to providing the highest level of care to our donors, surrogates, clients and their most treasured babies.

This week we have bid farewell to one of our old team member – Laxmi and have welcomed several new staff.

I would like to take the opportunity to introduce you to the wider team, most of whom you will probably never meet, who are crucial in the care of you, our clients, your egg donor, your surrogate(and their extended families) and your babies. The majority of these people work tirelessly in the background providing a high caliber of service that enables all of us at SCI to make your dreams of having a baby come to fruition.

Here are some of the staff who work in the back ground that make up part of the SCI team:-
1) Mr. Saurabh Kumar

Title: Proprietor, FCI.
Role: Mr. Kumar heads a team who is primarily responsible for the recruitment of surrogate mothers & egg donors. Mr. Kumar offers support and ensures that both surrogates and egg donors have all their needs met during the initial stages of treatment and after treatment has ceased. Mr. Kumar is the first point of contact for potential egg donors and surrogates and begins the screening process by explaining the process and ensuring that both donors and surrogates ( and their husbands’) understand what is required.

2) Dr. Ajay Sinha
Title : MD/ Physician
Role: Dr. Ajay Sinha evaluates all the surrogates & egg donors for Medical Fitness eprior to being accepted onto the program.

3) Dr. Atul Verma

Title: Clinical Psychologist
Role: Dr. Atul evaluates all the surrogates & donors prior to being accepted onto the program and provides psychological counselling as required. Dr. Atul spends a considerable amount of time ensuring that all parties understand the process and what is required and that both the surrogate and egg donor are able to give informed consent to proceed. Time is also spend with the surrogates’ husband to ensure that he is able to provide support to his wife during this process. Counselling is available to egg donors, surrogates and their husbands as needed.

4) Dr. Aarti & Dr. Malik

Title: OB/ GYN
Role: Dr. Aarti ( photo) & Dr. Malik work with me ( Dr. Shivani) at SCI as Senior Medical Officers reviewing of all the ongoing pregnancies. All ongoing pregnancies are reviewed formally on a fortnightly basis,although we meet daily with the wider team to review all cases.

5) Dr. Anil Malik

Title: Senior Surgeon
Role: Dr. Anil undertakes any surgical treatment, if required for the surrogates.

6) Dr. Usha Modi

Role: Dr. Modi undertakes all the pathology work and blood tests/ cultures. For our clients, Dr. Modi has one of the most important roles, her tests confirm that your surrogate is pregnant.

7) Dr. Dhir :

Title: Chief Anesthetist
Role: Those of you who choose to self cycle in Delhi will meet Dr. Dhir, who has the important task of providing anaesthetisia for all of our IVF cases / operative deliveries

8) Nurse Jija :

Profile : Nurse Jija is one of our IVF Nurses; she coordinates daily Injection & medication to all our Donors & Surrogates currently under treatment.

9) Rohan :

Title: Surrogate Support & Advocate
Role: Rohan sole role is to provide support and assistance to any surrogate who is admitted into hospital. No surrogate is ever left alone or isolated. Rohan will often communicate with the surrogate’s extended family and ensure that the surrogates concerns and wishes are addressed and understood by treating staff.

10) Jai Singh:

Title: landlord of Surrogate Housing
Role: Mr. Singh is the landlord of the Surrogate home. He is responsible for maintenance and security of all apartments and communal space.

Role : Poornima &Putul work in close association and manage a team of twenty social workers/support workers/cleaners. Their teams meet with surrogates and their families on a daily basis at the surrogate housing. They look after the daily up-keep of the studio apartments and ensure that the surrogate and their families have all their needs met. Fresh Food, drinkables and supplements are provided every second day for all in the apartment. If a surrogate chooses to have her children with her (often the case for younger children) then a second family member must be available to provide care.

In the past this has meant that the social workers have also assisted in facilitating the medical care and treatment of extended family members staying at the apartments, organised enrolment, uniforms and school runs for school aged children and assisted with on site child care, organised transport for extended families to visit and facilitated recreational activities.

The social workers’ work with the nurse to ensure that all meds are taken and the broader team to ensure that surrogates attend all appointments etc.
The Social Workers will monitor the well being of a surrogate on a daily basis and if there are any concerns then these are discussed with Dr. Shivani immediately.

12) Mr. S.K. Singh

Title: Purchasing Officer for SCI
Role : Mr. Singh ensures that we have all the medications required for the Surrogates and Donors on daily basis, as well as being responsible for all purchasing for the SCI team.

13) Parul Ojha

Title: Admin &Legal Manager
Role: Parul primarily role is in administration. Parul coordinates all the legal paper with SCI lawyer & also liaises with clients and ensures their queries are answered promptly.

14) Ritu Singh :

Title: Chief Admin Coordinator
Role: Ritu prepares and updates all our Donor & Surrogate profiles that are sent out to our IP’s through our International Client Managers. Ritu co-ordinates my diary and most importantly for IP’s ( works with Chandan) to books their surrogates for scans and sends copies to IP’s in a timely manner . If an IP has any questions regarding scans or any planned treatments, Ritu ensures that these questions are answered promptly.

15) Neha Yadav

Title: Chief of Secretarial Staff
Role : Neha role is to liaise with clients before IVF and during the pregnancy so that all are aware of ‘when’ and ‘where’. Most importantly for IP’s, Neha co-ordinates all the baby pick-up documentation for when it is time for your baby to come home.

16) Amit Kumar

Title: Accounts ManagerRole: Amit looks after day to day accounts of SCI.

17) Chandan Kumar :

Role: Chandan works with Ritu, Poornima, Putul, our doctors, social workers and drivers and the rest of the team to assist in coordinating daily appointments of the Surrogates & Donors with the Doctors for Scanning & Blood Tests.

18) Transport / Travel Assistants:

Role: We place the highest priority on the care of all our surrogates. This department looks after the daily transport needs of all the Surrogates & Donors. Through this team we are able to provide transport 24*7 to all our surrogates who only travel in SCI cars.

19) Pawan / Bhavani/ Rohit : SCI Office assistants

Role: As with any team, you need people who will jump in and do all the odd jobs that are required -These men do this for the SCI Team.

ISO Accreditation

Anyone in Health Care understands both the importance and the challenges of going through a Quality Management Systems ( QMS)registration and auditing process and all that goes with it;including having a culture of continuous quality improvement that is audited internally and externally.

It is with great delight that I share with you that we have been accredited with ISO for the maximum time period of 3 years.It is a commitment that we at SCI have made to continue to meet international standards of practice & care and that we are open to the scrutiny of the accrediting body. While we are all very excited about this as a service, the reality is that we will continue to provide the same high quality service & care to our clients, with no real observable change.

An ISO certificate is not a once-and-for-all award, but must be renewed at regular intervals recommended by the certification body, usually around three years. There are no grades of competence: either a company is certified (meaning that it is committed to the method and model of quality management described in the standard), or it is not

Our Experiences, thoughts from a new family

Hope you are well. We are doing just fine.

We left India Friday morning and came back to our house yesterday. At our house we had a party with our families and everyone were so happy that we finally made it back home.

Just wanted to share some of our experience and how we felt through the surrogacyjourney.

First a BIG thank you for all your help and support. You have been great. You are always positive, informative and you are answering our questions so quickly. We are very satisfied with both how you handle us IP´s and how you seem to handle the surrogates. The information about eggdonors and surrogates weren´t much but enough. The information you have sent throughout the pregnancy has been great. Always on time and for the most easy for us (dummies) to understand. Continue as now and you will have a long, good career. Your are wonderful!

During our visit in India we met Shilpi a lot and we adore her. She has been so wonderful to be with and she has helped us a lot. She is easy to talk to and she is eager to help and she wants everybody to feel comfortable. During Roberts visit at the hospital she was there many times, helping us and supported us. She is GREAT, hold on to her!

Alwina is doing just fine. Right now she has just been eating and lying in her bed getting some rest. She sleeps a lot and eats a lot.

Baby Boom continues...

It has been a very busy month at SCI with 10 international surrogacy babies born and the arrival of a group of Non- Indian Egg donors. We have had multiple families from Australia; the United States and Scandinavia ( including Alwina with her proud fathers) all in the process of collecting their much loved babies and have either returned home or are in the process of doing so.

Thankfully the process for going home has been streamlined and with some local support, IP's are able to navigate their local embassy/ consular and High Commission immigration requirements and the local bureaucracy and return home within two to three weeks.

Our concierge service managed by the very capable Shilpi have been run off their feet, assisting with local transport,accomodation, last minute shopping and transporting cots, sterilizers and baby baths for newborns across Delhi. It is also wonderful to see IP's who have only communicated via email and forums finally meet and support each other.

I have been privileged to observe so many wonderful moments from these parents,from the first time that they see and hold their baby; when they collect their baby from the hospital and just before they leave. One of the most touching was from a family from the United States, the mother told me:

“Ten months ago, i carried they cyptoshipper in my arms, now i am taking my baby back home”

For SCI, it doesn't get much better than that.

Aussie Baby Boom

THOUSANDS of foreigners are travelling to India in an attempt to conceive a child.

AFTER six miscarriages, years of failed in-vitro fertilisation treatments and endless queues at Australian and international adoption agencies, Megan Sorensen is finally expecting a baby this week, at age 43.

Like an anxious father-to-be, Sorensen (not her real name) will pace the corridors of New Delhi's Phoenix Hospital while a woman she met six days ago and knows only as Rani goes through childbirth for her.

Once delivered, the baby will almost immediately be handed over to Sorensen. And Rani, when she has recovered from her labour, will return to her own flat, her husband and two children.

In New Delhi the same process will be repeated several dozen times over for Australian couples before the year is out. Childless Australian couples -- heterosexual and gay -- are looking to Indian women who are prepared to rent out their wombs for the chance to improve the lives and fortunes of their own families.

Delhi fertility specialist Shivani Sachdev Gour says she has seen an explosion of Australian clients as word of her service has spread through the community of couples exploring surrogacy options.

Since the first Australian couple walked through the door of her low-key clinic last year, she now sees at least 10 new Australians every month who have travelled to India -- many of them for the first time -- in a last-ditch effort to conceive a child.

"Of 100 surrogates on my books, 55 are pregnant and more than 50 per cent of those children will be born Australian babies," Gour says. "Most of the [commissioning parents] have done IVF in Australia and been advised by their specialists that surrogacy is their best option."

Her first successful Australian birth came just three weeks ago, to a single man who came to India for two days of treatment, gave a sperm sample on the day the donor eggs were collected, and nine months later collected his baby.

Unlike some Indian fertility specialists, Gour says helping aspiring single or gay parents conceive a child poses no ethical dilemmas for her. She's vehement when confronted with the criticism that using a poor, often ill-educated woman to incubate a wealthy woman's child amounts to exploitation. "Just because the [surrogate] is poor it doesn't mean she's not allowed to make her own decisions," she says. "The Supreme Court of India says surrogacy is an industry."

Indeed it is. More than 100 operators turned over an estimated $US445 million ($514m) last year.

But, for some, India's reputation as the world's baby factory for foreign women unable, or unwilling, to pay Western surrogate fees is a grotesque commercialisation of the reproductive system.

Sorensen has heard all the arguments before. "People say really nasty things, that we're selfish for wanting our own child," she says. "What really gets me is when they accuse us of going to India to buy a baby like it's an easy process. It's not." She calculates the whole process -- including one failed effort and one miscarriage -- will have cost more than $90,000 by the time their baby is delivered. Of that, Rani will receive $5000.

While thousands of foreign children have been delivered by Indians without incident, several cases -- including the death of a surrogate during childbirth last year -- have scarred the industry. The woman, a second wife, was pressured by her husband to become a surrogate to earn more money for the family. And in 2008 the industry faced a scandal when a Japanese couple broke up before their child was born, leaving the baby in danger of becoming India's first surrogate orphan.

India's minister for women and child development Renuka Chowdhury warned two years ago: "We do not want surrogacy to become unfettered like the organ trade. We need to put a regulatory authority in place."

Draft legislation governing the entire assisted reproductive industry -- IVF, sperm and egg donations and surrogacy -- is to be debated in parliament within months. If passed, it will legalise surrogacy services for couples and single people and provide a loophole for gay couples by allowing one partner to register as a single parent on the birth certificate.

Surrogacy clinics will be forbidden from recruiting and acting for surrogate mothers, who will instead be represented by a third party. The law also will forbid a commercial surrogate from carrying more than five babies in her lifetime, including her own.

Australian law further stipulates that a child born overseas of a surrogate mother must have a DNA link to at least one of the commissioning parents.

Gautam Allahbadia, who helped draft the bill, says he expects it to pass with little trouble after five years of debate and amendments.

The Mumbai-based fertility specialist says India is an ideal surrogacy destination; Indian women rarely drink or smoke and the country offers "First World medical services at Third World prices".

But National Federation of Indian Women president Annie Raja fears the new law will lead to the exploitation of more poor and lower caste women. "This country has one of the highest maternal and infant mortality rates. Nobody is thinking about the mental or physical health of these women. It's all about money," she says.

At Gour's clinic money seems the furthest thing from Sorensen's mind as she clucks over her young surrogate. Sorensen is ebullient and awestruck. Rani seems overwhelmed. Both women are close to tears. Through a translator Rani says she is "a little nervous" about the labour and concedes giving up the baby she has carried for 37 weeks, but has no biological link to, will probably be painful.

But she says: "It's a few hours of sadness for me and a lifetime of happiness for Sorensen."

Asked if she would do it again she doesn't hesitate; "One hundred per cent."

But she looks uncomfortable when asked to explain how being a surrogate will improve her family's fortunes. For 10 months Rani has had a driver, maid and food delivery service, her rent and all family medical bills paid. When the baby is delivered she will receive 200,000 rupees ($4981), one-tenth the price of the most cut-rate US surrogate. For many Indian surrogate mothers all the attention that comes with carrying a wealthy woman's baby ends soon after the child is delivered. But Sorensen says she is determined to make a difference to Rani's life by helping her buy a home and paying for her children's education. "I feel very maternal towards Rani," she says. "She's part of our baby-making team."

Deene Vishnu Kanth is our highly skilled embryologist with many years experience. I was amused by his description of his work to a client recently in which he stated his role was to look for the best sperm under the microscope and introduce it to the healthiest egg and then wait for the miracle of fertilization.

Devender Kumar is our Junior Embryologist and more than likely is the person that most of our fathers will meet (besides me of course!). He recently told a father-to be that his job was to make sure that nothing got mixed up. Everyone laughed at this, but later on i watched on with amusement as he corrected a nervous father's spelling of his name and date of birth.

Good embryologists are crucial to any team undertaking surrogacy and i am pleased and proud to have two such excellent professionals as part of SCI. Both of these men are a key component to our sucess.

He’s a 30-year-old software engineer, who is planning to start a family soon. But he often thinks about the children he has already helped bring into the world -- as a sperm donor.

"One of the main reasons I became a donor is that I believe I am a good person, and wanted to help create good people,” says the Mumbaiite, who donated ten samples as an engineering student. And even though he didn’t get to hear any Father’s Day greetings last week, he says, “I hope I have passed on my good values to the children, wherever they may be.”

The face of the sperm donor has changed dramatically in the last few years. “We had a hard time finding donors in the past, but we now get phone calls and emails from people wanting to donate,” says Dr Anjali Malpani, director of Mumbai’s Malpani Infertility Clinic.

Sperm banks are reporting an increasing incidence of “high IQ people” coming forward as donors. “We have a varied lot -- CEOs, MBAs, accountants, engineers, executives, paramedics and secretaries,” says Malpani.

Dr Iqbal Mehdi, director of semen bank Cryo Lab, has student donors from IIT, JNU, MAMC, DU and IGNOU. Not surprisingly, the IIT samples are in great demand.

Medical students are the staple of most sperm banks. Says infertility specialist Dr Anoop Gupta of Delhi IVF and Fertility Research Centre, “About fifty per cent of our donors are medical students. We take such donors because they understand exactly how their donation can make a difference to others, and aren’t commercially motivated.”

At Rs 300-600 per sample, money isn’t an attraction, says Kapil (name changed), also a software engineer, who has been a donor for the last three months. “I won’t waste six hours a week commuting from Noida to Delhi to get the money. I could earn much more in that much time if I start a computer coaching institute,” he says.

Fertility specialist Dr Shivani Sachdev-Gour of Phoenix Hospital was surprised to receive a request recently from a 22-year-old working with a leading audit, tax and consulting firm, to donate only on the condition that there’s no money involved.

“I will be doing very little to fundamentally change the lives of ten families (as per ICMR guidelines, semen of one donor cannot be used for more than ten successful pregnancies),” says the aspiring donor, who is also an organ and blood donor.

When he has his own children, will he tell them about the children born out of his donations? “Definitely. Among the values I would like to inculcate in my children would be to help fellow human beings. I would like to lead by example,” he says.

Counselling the donor is important to ensure they don’t get attached to the children. “We tell potential donors that one in six couples is now infertile, compared to one in ten earlier. In many cases, male infertility is the problem. Sperm donors feel happy to give such couples a chance to be parents,” says Dr Sohani Verma, in-charge of the IVF unit at Indraprastha Apollo hospital.

Donor anonymity is mandatory in India, though it has been done away with in some sperm banks abroad. Says Dr Gour, “No donor wants any liabilities. You don’t want ten children knocking on your door.”

As Ajay (name changed), a 32-year-old paramedic who is a frequent donor puts it, “Even if the law permitted me, I wouldn’t want to meet children born from my donation. I may be their genetic father, but the man they consider their father may feel bad on seeing me.”

On a hot summer’s day in the tony south Delhi neighbourhood of Greater Kailash-I, Norwegians Freya Reintoff* and her husband Arian Rakke* carefully sifted through dozens of pictures of Indian women.
Reintoff (36) and Rakke (37) looked for “a happy, open face”, characteristics they hope will help them in their search for a healthier, more intelligent baby.
The couple live in the town of Holmestrand (population: less than 10,000) on the east coast of Norway, and are one of many Caucasians making the long trip to India to treat infertility by opting for donor eggs from ethnic Indian women.
Here’s why: multiracial babies are perceived to be healthier and more intelligent.
“We researched on the Internet and chose an Indian donor because it does not really matter to us whether the baby looks like us or not,” said Reintoff, an English teacher who has barely managed to get over the cultural shock of her first visit to India. “What’s important is that the baby is healthy.”
Reintoff — a blue-eyed, blonde — said she wasn’t looking for a designer baby, just a smart, happy one.
“India is very different but the people are beautiful and the food is great,” said Rakke.
Egg donation is an infertility treatment in which eggs taken from a donor with her consent are fertilised with sperm from the husband. The embryo thus created is implanted into the womb of the childless woman. A legal agreement is signed; the donor’s identity is protected and the donor gives up all rights to the baby.
Each month, four to five such couples looking for Indian donor eggs visit Dr Shivani Sachdev, infertility expert at Phoenix Hospital, where Reintoff and Rakke went.
Medical tourism companies confirm the number of Caucasians getting treated for infertility in India has shot up over the past year.
“People travelling from the US, Canada and Europe have doubled over the past year,” said Kaushik Shukla, COO of We Care, a Mumbai-based medical tourism and treatment-packaging company that offers IVF, egg donor, surrogacy and infertility treatment solutions.
“We have tied up with clinics in Delhi, Mumbai, Bangalore and Chennai where we send them for treatment.”
The perception is that multiracial children are healthier and brighter, said Dr Sachdev.
“I am currently using Indian donor eggs to treat four Caucasian couples — two British, one American and one German,” she said. “The American couple chose a girl who looked very ethnically Indian, and another Norwegian couple were here today looking for Indian donors.”
Not all Caucasians, however, want Indian donors.
Elizabeth Burns*, 50, a Canadian married to an Indian in Kolkata, wants a Caucasian donor who resembles her.
“I always thought race didn’t matter but when I got down to actual decision-making, very strong instincts kicked in,” said Burns. “I wanted to choose a donor who looked like me and the women in my family. Since Caucasian donors are limited here, I’ll choose a donor over the Internet and have the egg shipped to India.”
“Apart from similar genetic backgrounds, I am going to consider the health, educational and height of the donor,” said Burns.
Under India’s National Guidelines for Accreditation, Supervision & Regulation of ART Clinics in India, anonymous donation is allowed from donors who are “healthy (medical and psychological examination, screening for STDs, and absence of HIV antibodies) women in the age group of 18-35 years”.
Unlike Sachdev, Dr Hrishikesh Pai, infertility expert with Mumbai’s Lilavati Hospital and Fortis La Femme in Delhi, does not show photographs of donors to recipients. Instead, he provides a detailed description of the donors, including colour of the eyes, hair and skin. “People don’t care so much about looks as 50 per cent of the genetic matter is the husband’s,” said Dr Pai.

Last year’s sealing drive took away her husband’s catering unit. This October, recession cost Anita her retail job.

Christmas, though, may finally bring some cheer — and money — to this 26- yearold retail management postgraduate.

No, she hasn't found another job. But she will raise money by renting out her womb to an American woman who is flying in to India to start the procedure next week.

"I have an MBA degree and have done several computer courses. But I didn’t look for another job after losing the one that I had. The job market is just too tight," Anita says.

Easy money in hard times: Educated and middle-class women affected by meltdown are turning surrogates or egg donors
The mother of a two- year- old hopes to be pregnant by January 2009. She'll be paid Rs 2.75 lakh, along with all expenses incurred on groceries and medicines, for the next nine months.

"If she hadn’t lost her job, she wouldn’t have bothered to do this. You don’t generally get such well- educated surrogates or donors, unless they are from the paramedic profession," says Dr Shivani Sachdev Gour, a fertility expert at Phoenix Hospital, Greater Kailash- I, where Anita will undergo the procedure.

It's win- win for Gour's American patient as well. She'd have had to shell out $40,000-50,000 (Rs 20-25 lakh) to a surrogate in the US. The recession is fuelling a baby harvest. It's evident from a visit to the Delhi In-Vitro Fertilisation (IVF) and Fertility Research Centre at Bengali Market.

Reeta, a software engineer, has taken time off from her IT firm in Gurgaon to donate eggs to an infertile couple being treated at the centre. She decided to do this after her husband, a software engineer in the US, returned to India after being laid off.

Reeta's eggs go for a premium, thanks to her high IQ profile, and she makes Rs 40,000 to Rs 50,000 each time. "My husband is trying to raise money to open his own software training institute. I want to help him, as well as help infertile couples have babies," she says.

Confirming the trend, Dr Anoop Gupta, Delhi IVF's infertility specialist, says: "In the last two months, we have had seven or eight couples walking in with prospective egg donors and surrogates who are all white-collared workers affected by the economic crisis. People facing recession know about this opportunity that will help them and also assist infertile couples." In Gujarat's Anand — India's surrogacy capital — Kaival Hospital's infertility specialist Nayna H. Patel says the number of educated and middle-class surrogates and donors from towns such as Vadodara has shot up by 15-20 per cent.

"Many of these women come after losing money in the share market or after either they themselves or their husbands lose their job," Patel adds.

One of the surrogates under her care is a 26-year-old woman who has an LLB and a BCom degree. The woman turned to surrogacy after her husband was rendered jobless because of the economic downturn.

For Mayur Vihar housewifeturned- tutor Mandakini, becoming an egg donor came as an alternative to suicide, which she was contemplating after her husband, a sound engineer, lost his Rs 40,000-a-month job.

"We moved from a two-bedroom home to a one-room set with a kitchen," Mandakini recalls. "My son’s marks fell from 96 per cent to 64 per cent because we couldn't afford his tuitions any more. We didn't have the money to pay his school fees. I had gone to a chemist to buy poison, but didn't know what to get. That's when I saw an ad for a donor in a women’s magazine." Egg donation is a long- drawn process, involving 9-10 days of injections, and the subsequent removal of eggs under general anaesthesia. Specialists say the procedure is safe.

"There are no cuts and the entire procedure is done with the help of a needle guided by ultrasound," assures Dr Deeksha' assists Dr Gupta of Delhi IVF. Unmarried women are turned away from donation, for it leads to the tearing of the hymen during the medical examination, which isn’t held in a good light in many traditional homes.

Dr Gupta recently refused an infertile couple who came with a prospective donor — a laid-off airhostess — because she was unmarried. "We take women who are married and have already had a child. They prove to be fertile," he says.

In some states in the US, even college students donate eggs to pay their tuition fees. "Depending student could receive anything between $5,000 (Rs 2.4 lakh) and $30,000 (Rs 14.5 lakh)," says Dr Sulochana Gunasheela, who runs her own IVF centre in Bangalore.

"Models and women with high IQ invite online bids for their gametes." All this may seem somewhat futuristic, but the way urban India is moving, are we likely to see educated middle-class women catching up with their US counterparts? "If not now, possibly some time in the foreseeable future," says Dr Gunasheela.
